Non-healing wounds often result from underlying medical conditions, such as diabetes, heart disease, or cancer. Bacterial infections and lifestyle factors, such as smoking, poor nutrition, and obesity, can also contribute to a wound that won't heal. Wound Debridement
The first step in any wound healing treatment is debridement—removing dead and damaged tissues from a wound and any contaminants or foreign bodies.
Skin Graft Surgery
Some wounds are too large to suture together. In these instances, a surgeon may transplant healthy skin from another area of the body to help facilitate wound healing. If the two top layers of skin are harvested (the epidermis and the dermis), the procedure is known as a split-thickness skin graft. If your surgeon removes the entirety of the skin from the donor site, including blood vessels, it is known as a full-thickness skin graft.
Flap Surgery
Flap surgery involves removing healthy tissues (such as skin, fat, and muscle) from one area of the body and relocating them to another. Flap surgery is often used for reconstructive purposes since it can restore volume and shape to an area. Depending on your wound, your wound care specialist may perform a pedicled flap or free flap procedure.
With a pedicled flap, the tissues remain partly attached at the donor site and are usually tunneled under the skin to their new location. Pedicled flaps remain attached to their original blood supply. If there isn't an acceptable donor site nearby, you may be a candidate for a free flap surgery.
With free flap surgery, the tissues are removed completely from the donor site and then reattached at the site of the wound. An experienced wound care surgeon will use microsurgical techniques to reconnect blood vessels, muscle, skin, nerves, and other tissues.
