Plastic Surgery Referral

The Institute for Advanced Reconstruction respects the relationship you have with your patients. When you refer a patient to us, we act as an extension of your care and recognize that the patient still looks to you as their primary doctor.

Our care team is sensitive to the complex emotions that many patients who need breast reconstruction experience. We promise to work as efficiently as possible to determine the best course of treatment for your patient. Referring physicians can expect open communication, transparency, and prompt follow-up, including visit summaries, test results, surgery summaries, and discharge notes.

How to Refer a Patient

If you have a plastic surgery referral, please complete our patient referral form. You can also refer a patient directly by phone at 1.855.980.6981.

In addition to your contact information, our clinical team will need the following information:

  • The patient’s demographic information and insurance (if available)
  • MRI Disc of the affected area
  • Relevant test results and diagnoses
  • Your most recent progress notes

Our friendly patient intake team will work with your patient to complete the registration process and to schedule an appointment. 

Insurance

We accept many major insurance carriers. Before scheduling an appointment, please get in touch with your insurance carrier to confirm that your provider is in-network, as each physician has their own accepted insurance and hospital affiliations.

Many of our physicians also accept Medicare. Our billing department will help you confirm your coverage before your visit.

For patients that require additional financial support, we also offer financial aid through CareCredit.
